Dear Homeowner;

    Thank you for inquiring about our services at A. Macchione Brothers. Enclosed is an estimate for your project as well as the information which will help you choose the right contractor to complete the job properly. If you agree to the price quoted and would like A. Macchione Brothers to do the job for you, these are the steps you have to take to insure that the job gets done quickly and efficiently:

  1. Sign and date the bottom of the estimate. Clearly print your name and fill in the total cost of the job.
  2. Fill out and sign the Certificate of Capital Improvement (W-8). This form is required by the government so that you do not have to pay sales tax on home improvement jobs. This does not include any repairs, patching or sealcoating.
  3. Each town has different regulations regarding whether a permit is necessary and how to obtain one. After we receive your signed estimate, we will inform you as to whether or not your town requires a permit. If your town does require a permit, we will need a copy of your property Survey from you. This is the Survey that you received at time of closing. If you no longer have a copy of your Survey, you can contact your closing attorney, bank or mortgage company or surveyor. We take care of filling out any applications and paperwork and obtaining the actual permit. The cost varies from town to town and is added to the final cost of your job. Most towns charge between $15 and $75 for a driveway permit, and aprons or sidewalks are additional.
  4. If you have access to a fax machine, fax all of the completed forms to us. Our Fax number is (201) 487-5325. Once we get all of your forms, we can then contact you to schedule your job. If you do not have access to a fax machine, you can either mail the completed forms to us or call the office. When you call, please have the estimate form with you so we can get all the necessary information from you to get the job set up properly. Scheduling a job usually takes anywhere from one to three weeks, depending on weather, how busy we are, and other job running smoothly.
  5. If you are paying by check, a bill will be sent to you after the job is completed. We do not require any money prior to the job being started; however, we ask that you forward payment immediately upon receiving your bill. Our payment terms are upon completion. Even though you do not have to personally be home when the job is being done, we do require certain things of you:

    a)    On the day we start to rip out the driveway, of course you would have to move all vehicles from the driveway. The vehicles can be driven and parked on the driveway once the driveway has been stoned.

    b)    On the day we come to pave the driveway, you have to remove all vehicles from the driveway. The vehicles must stay off the driveway for four (4) to five(5) days after we are finished. We also require you to either leave your garage door open or unlocked for us. If you have a garage door opener, we can make arrangements with you for access to the opener.
